Healthcare Providers

We accept the following insurance types to cover our telepsychology services:

• Auto Insurance / Personal Injury Protection (PIP): Covers psychological and neuropsychological services following a motor vehicle accident, regardless of fault.
• No-Fault Auto Insurance: Provides immediate coverage for medical and psychological treatment after an auto accident.
• Medical Payment Benefits (MedPay): An add-on to auto insurance that covers medical and psychological expenses up to the policy limit.
• Premises Liability MedPay: Covers psychological treatment for patients injured on someone else's property, including slip and fall accidents.

Please include the insurance company name, claim number, and policy number when submitting your referral.

Referring a patient to MyPhoneTherapies is straightforward:

• Complete the Online Referral Form Below: Fill out the secure referral form on this page and upload any pertinent medical records directly through the form.
• Email Documents: You can also send medical records and supporting documentation to referrals@myphonetherapies.com. Include all pertinent demographic information, insurance details with claim number and policy number, and attorney information.
• Patient Registration: Please have the patient complete our online patient registration.
• We Handle the Rest: Once we receive the referral and verify insurance coverage, we will contact the patient directly to schedule their first appointment — typically within 24 hours.

A referral from an attorney is not required; however, if the patient has legal representation, please include their attorney's contact information.
